Option 1 Include any elected or appointed leadership position you held in the last three years. List the name of the organization or group for which you held the leadership role and include your elected or appointed title/position. Include the duration of your position/title and a list of your accomplishments and responsibilities in the position. Option 2 List your significant academic achievements/honors/awards over the last three years. Indicate the appropriate year beside each honor. Option 3 For each event or organization you volunteered with in the last three years, list the name of the event/organization, the duration and total hours you volunteered, and what your responsibilities or accomplishments were. Option 4 For each extracurricular activity you participated in over the last three years, list the name of the organization/team/group, the duration of your participation and your responsibilities or accomplishments as a member. Option 5 For each paid position you’ve held in the last three years, list the name of the employer, the title of your position, the duration and total hours you worked and a list of your responsibilities or accomplishments.
